What is a Sliding Fee Scale?

Sliding Fee Scale for Medical, Dental, and Behavioral Health Services
A sliding fee scale is a program that adjusts the cost of care based on a patient’s household income and family size. It helps ensure that everyone in our community can receive essential medical, dental, and behavioral health services, even if they are uninsured, underinsured, or experiencing financial hardship.

Medical Services
Our sliding fee scale helps make primary and preventive medical care more affordable. This includes a wide range of services including checkups, chronic disease management, and immunizations. Patients with lower household incomes pay a reduced fee. Insurance is billed when available, but discounts apply to the remaining balance.

What this means for you:
  • Lower out‑of‑pocket costs for essential medical visits
  • No denial of care due to inability to pay
  • Clear, consistent pricing based on income guidelines
Dental Services
Dental care is an important part of overall health, and cost should not prevent anyone from receiving treatment. The sliding fee scale applies to preventive and restorative dental services, including cleanings, X‑rays, fillings, and extractions.

What this means for you:
  • Reduced fees for common dental services
  • Upfront estimates based on your fee scale level
  • Support for patients without dental insurance
Behavioral Health Services
Mental and emotional well‑being are essential to overall health. The sliding fee scale helps make counseling, therapy, and psychiatric services more accessible.

What this means for you:
  • Lower-cost behavioral health appointments
  • Access to care even without insurance
  • Reduced financial stress during times when support is needed most
How Eligibility Is Determined
Your placement on the sliding fee scale is based on:
  • Household income
  • Number of people in your household
  • Proof of income, such as recent pay stubs or tax documents

All information is kept confidential and used only to determine your fee level.

Why the Sliding Fee Scale Matters
This program helps remove financial barriers to care and supports a healthier community by:
  • Making preventive and ongoing care more affordable
  • Reducing delays in treatment
  • Ensuring equitable access to medical, dental, and behavioral health services
  • Supporting individuals and families during financial challenges
