You can see your vaccination history by using the Illinois Department of Public Health (IDPH) self-service portal, Vax Verify, or by filling out and submitting a request to IDPH. Please visit dph.illinois.gov for more information. If you don’t have a copy of your childhood vaccination records at home, call your healthcare provider or medical clinic where you received a vaccination shot. You can also check with your high school/college or previous employers that may have required immunizations.
The Macoupin County Public Health Department only has access to medical, dental, and immunization records for people who have received medical services, dental services, or immunization services from MCPHD.

If you’ve received treatment through MCPHD for a Substance Use Disorder (SUD), your records are protected by a special federal law called 42 CFR Part 2. This law gives you extra privacy protections beyond regular medical privacy laws like HIPAA. To share your SUD treatment records, you’ll need to sign a Part 2-specific release form—not just a standard HIPAA form. This form ensures you understand:
- Who will get your information
- Why it’s being shared
- That you can change your mind at any time
