COVID-19: What You Need to Know

MCPHD has established a COVID-19 hotline for area residents. If you have questions about COVID-19, please call 1-217-313-5078 to speak to one of our Emergency Command Center team members. You may also email your questions to mcphd@mcphd.net. If you are experiencing symptoms (fever, cough, shortness of breath), contact your primary care physician. If you do not have a primary care physician, call 217-854-3692 and ask for the Public Health Nurse.

To sign up for one of our upcoming COVID-19 vaccine clinics, click on a date below.

You must go through the entire registration process at which time you will receive a confirmation email. If you cannot self-register online, call our COVID19 command center helpline at 217-313-5078 for assistance.

This clinic is for ILLINOIS RESIDENTS ONLY who are 16 YEARS and OLDER.

When you arrive at the door, you must show an Illinois driver's license or Illinois photo ID (showing your name and address) to confirm residency. If you cannot show Illinois residency, you will not receive a vaccine.
